Saying someone has a “third leg” is a euphemism for saying they have a really big penis, hence when adapting it to ponies it turns into “fifth” leg. I’ve never been sure what the criteria for either tag is exactly apart from an impossibly large penis. The only thing I can think of is the character’s penis is hanging low in the image, hence it looking like an extra leg, but that seems to be inconsistent.

Derpi defines them as “a penis reaching down as long as their legs (usually touching the floor),” and “an anthro or human character has a penis about as long as their legs.”
Personally, I think that’s a little too literal, and tend to add the tag to any where it’s disproportionate, but not (much) longer than the character’s torso.

As a point of comparison, the breasts go big (reasonably big) -> huge (comparable to owner’s head) -> impossibly large (comparable to owner’s ribcage) -> hyper (comparable to the whole torso and larger).
Yes, there is meaningful difference. Same as there’s meaningful difference between “big penis” and “huge penis” despite what derpitards decided. Lemme draw a rough guideline…

He uses Blender instead of SFM, and better models than the usual plastic SFM dolls.
