Torture, yes I agree.
The wheel is a “wartenberg wheel” and is pointy but not sharp enough to penetrate skin. It’s originally designed to gauge a patient’s sense of feel and touch and to find pain and nerve damage. BDSM people started using it as a tool to torment their subs either by light pressure resulting in tickling, or hard pressure being mildly painful.

I’m human! So what?
Is he really thinking this is ticklish? I’m pretty sure this is actual torture and not just torture by tickling but he literally has sharp fingernails scratching on his feet and the other foot is getting cut with what looks like a surgical saw.
