“Spoiled Rich is worried that Filthy doesn’t find her that attractive anymore, especially when she thinks she sees him eyeing other women like that young tart Applejack with her big boobs and bigger ass.
 
Well, two can play at that game.  
She goes to have a new kind of plastic surgery done to give her the large breasts she needs to win her husband back.  
It involves a magical infusion of enchanted fluid that makes the breasts full and round as the fluid converts to actual flesh.
 
Even though it is a brand new technique she does it anyway.  
Unfortunately, once she is under anesthesia and hooked up to the infusion pump, something happens to distract the nurse who is supposed to be monitoring the 15 minute process.
